Scheduling an appointment for Issue Focused Ministry requires commitment and follow-through. These three hours can profoundly change your life as God sets you free from the root causes of an on-going burden in your life.

Your personal questionnaire will be assigned to a certified Issue Focused Minister who will prayerfully form a ministry plan that is unique to your issue.

All statements that you make to the minister (and to any other assistant present) are of a confidential nature, including all written information.

Women are assigned to women teams and men to men teams.

Your minister/ministry team will spend approximately three hours with you ministering to the four problem areas related to your issue. We will apply foundational truths to each of these open doors as we listen to God for counsel and direction.

Your minister/ministry team will provide you with “homework” and “walking it out” tools.

I will spend time in heart preparation prayer prior to my scheduled session.

I agree to meditate on the Godly beliefs that my minister helps me to develop for a period of 30 days following ministry.

You may either print the form and fill it in by hand or you may fill out the form digitally on your computer. If filling digitally, open the downloaded PDF in Adobe Acrobat Reader, save it to your computer with a unique name, then proceed to complete it in its entirety, saving frequently.

Step 2
Submit your completed questionnaire by email to rtf.npcf@protonmail.com or send a printed version to Restoring the Foundation Ministry, c/o New Promise Christian Fellowship, 12851 N 19th Ave, Phoenix AZ 85029.
Step 3
Normally within two days, you will receive confirmation that your questionnaire has been received and reviewed. You will receive a confirmation email with a proposed ministry date.
Once you have received your confirmation email, you should submit your payment by clicking on the "Buy Now" Paypal button below.

Finally:
You will receive a Receiver’s Guide prior to your session to help you become familiar with the format and the prayers you will pray. This is a time to ask the Lord to prepare your heart. We will be praying for you!

We reserve the right to deny an applicant ministry if we deem the issue is beyond our skill and training level. We are not trained to minister to deep occult involvement, SRA or such issues as multiple personality disorder.
